(2) Correction, addition, and deletion
You may request us to correct, add, or delete your personal information following the procedures outlined in (4) below. In such cases, we will conduct an investigation without delay to the extent necessary to achieve the purpose of use, and based on the results, we will correct, add, or delete the personal information to the extent permitted by the Act on the Protection of Personal Information.
(3) Suspension of use or suspension of third-party provision
You may request to suspend the use of your personal information or its provision to a third party following the procedures outlined in (4) below. In such cases, we will respond appropriately per the Act on the Protection of Personal Information.

We will provide you with our designated request form necessary for disclosure. Please fill in the required information on this form, enclose the following [identification documents], and mail it to the address specified by our company (Tokyo Sante Support Co., Ltd., Personal Information Disclosure Request Desk, 3F Okada Building, 109 Yaraicho, Shinjuku-ku, Tokyo 162-0805), (postage not included). In addition, a fee of 1,000 yen will be charged for each request for notification of the purpose of use and disclosure of personal information or records of provision to a third party, so please transfer the fee to the bank account listed on the request form.
